Demo instances of integrated systems OpenMRS and Bahmni

Summary
Different openIMIS integrations were developed in the past, some of them being production ready (Bahmni) and some not (OpenMRS). This integration was done through openIMIS FHIR STU3 module. Later, FHIR R4 module was developed (through AI project) and is currently improved (through E0 project). Because FHIR STU3 module is not maintained anymore, the integration need to be redeveloped to support FHIR R4 module. Moreover, demo instances are to be created on the demo server.
